  • Long Term Care Scorecard. This AARP national scorecard on aging services provides benchmarks for such measures as effective transitions, support for family caregivers and affordability and access.

Professional Development

  • Rochester Educational Opportunity Center. With the support of the College at Brockport and the SUNY Center for Academic and Workforce Development, this center offers workforce development programs for a variety of positions including pharmacy technician, LPN, CNA and more. The center also provides support services leading to employment opportunities, access to further education, personal growth and development.
  • Home Health Aide Training. This three week course through the University of Rochester Medicine Home Care provides training in personal care and other living assistance.

  • University of Rochester Finger Lakes Geriatric Workforce Enhancement Program. This partnership among the University of Rochester, Lifespan, FLPPS and Ithaca College seeks to improve patient and family engagement for older adults.  The program also aims to meet training needs related to Alzheimer’s disease and related dementia.

  • Community Health Worker Training. This online training through MCD Public Health includes training in the management of chronic conditions such as diabetes, asthma and high blood pressure.
  • Western New York Rural Area Health Education Center (WNY R-AHEC). This Warsaw, NY nonprofit builds community partnerships to educate and place health care professionals in underserved areas. The center’s work includes developing career ladders, coordinating student training opportunities and providing career path development for local youth at all school levels.
  • Advancement Training for Healthcare Occupations (ATHOS). These regional courses include training for community health workers, cultural competency/health literacy, motivational interviewing and professionalism. Download their 2017 flyer

  • Ibero American Action League Language and Cultural Services program. The program includes medical interpreting, translations, Spanish classes and cultural competence training.
  • Opioid Overdose Prevention Program As a state Department of Health Registered Opioid Overdose Prevention Program, Trillium Health provides on-site Opioid Overdose Prevention (OOP) training and naloxone kit distribution the fourth Saturday of every month at Trillium Health’s main location, 259 Monroe Avenue, Rochester NY 14607.  Trillium can also bring the training to other organizations free of charge.

  • Open Spaces Training Trillium Health offers both short and full-day LGBT cultural competency trainings (known as Open Space Training) to all health and human service providers. The trainings are tailored to fit each organization’s needs, with the overall goal of increasing the cultural competency and skillsets of health and human services staff members as it relates to working with the LGBT community. All trainings are funded by New York State and free of charge.

  • The National Health Service Corps training month. The NHSC offers financial and other support to primary care providers and sites in underserved communities.

  •  The Mental Health Association provides numerous culturally competent education programs.


  • Guidelines for Core Curriculum to train Care Coordination Workers. Prepared by the DSRIP/SIM Workforce Workgroup, this curriculum provides content on key background and content around care coordination, including interdisciplinary teams, the social determinants of health, management of chronic disease, cultural competence and health information technology.
  • Finger Lakes Works. Authorized by the Workforce Investment Act of 1998, the program links employers and training opportunities with job seekers through job fairs, postings, workshops and other outreach.

Work flow

Common Ground Health Resources. This library provides a wealth of resources related to work flow arrange by topics such as end of life care, transitions of care and patient activations.

Practice Transformation. Common Ground Health provides free consulting for qualified medical practices on a variety of areas, including leadership development, patient engagement, care coordination, preparing for value-based payments and using data to improve patient outcomes.

Technology solutions

  • Assistive Technology. This University of Rochester Medicine Home Care program makes available an assortment of assistive technology devices to help keep patients and their families safe at home. The program products include cellular devices, GPS devices and fall detection pendants.

  • Home Telehealth. This University of Rochester provides clinicians with the ability to monitor and measure patient health data and information such as blood pressure, pulse, weight and other important vital signs remotely.

  • Finger Lakes degee programs in healthcare Updated in August 2017, this spreadsheet lists health related educational degrees offered by 18 educational institutions in the region.

  • The Center for Health Workforce Studies assists health, professional, and educational organizations, policy makers, planners, and others to understand issues related to the supply, demand, distribution and use of health workers. The center’s goal is to provide high quality, relevant research to help lead to informed decision making. The center’s New York Health Data Guide provides information on population demographics, health behaviors, health outcomes, as well as the health care system and workforce. View the guide here.

    Economic and Workforce Development Center Through extensive research of local labor market intelligence, this center identifies career areas in need of workers and creates the academic, career and educational programming needed to train those workers.  The center is a part of Monroe Community College’s division of Economic Development and Innovative Workforce Services.

Recruitment & retention

  • ROC-HPOG An Action for a Better Community program, this network of partners offers pre-training activities for participants who need to improve basic academic skills or English language proficiency before entering health occupational training. Training is organized as a series of manageable steps leading to successively higher credentials and employment opportunities. Programs prepare participants for college, and/or industry-recognized credentials.

NYSDOH Regulatory Modernization Initiative
Launched in the fall 2017, this Department of Health effort gathers feedback on health regulations that are inhibiting the delivery of care as technologies and practices continue to evolve. C‚Äčommon Ground Health is providing the Department of Health with recommendations from our region on issues where there is consensus.

  • Common Ground Health supported the proposed Limited Integrated License provided that the state provide clarification on reinbursement policies and other specifics. Read the Common Ground Health recommendations here.

Authorize Standing Orders The Medicaid Redesign's work group for workforce flexibility and change of scope of practice recommends authorizing the use of practice protocols, or “standing orders,” to permit nurses in specified situations in emergency departments or other settings. The group noted that such standing orders could be used in matters such as mandatory administration of newborn prophylaxis and other immunizations, and treatment protocols and regimens in emergency departments for conditions such as acute asthma, acute myocardial infarction and stroke. Education Law §§ 6909 and 6527 would need to be amended to permit the use of “standing orders” for specified regimens. Details here.

Expand Scope of Practice for Licensed Practical Nurses Education Law § 6902, which sets forth the scope of practice for Licensed Practical Nurses (LPNs), does not permit LPNs to provide patient assessment/triage services. Potentially, the statute should be amended to permit LPNs to perform specific tasks, under the direction of a licensed professional registered nurse, in certain settings. This change would increase flexibility and access. Details here.
