Making new connections is the goal of a new state grant that has been awarded to our region.

S2AY Rural Health Network has received a $2.5 million grant from the state Department of Health to create the Upstate Community Based Organizations Consortium, which will help providers connect patients with non-clinical services.

S2AY will work with CBOs and the Performing Provider Systems across upstate New York to build relationships and referral networks as part of our region’s effort to redesign the way Medicaid services are delivered under DSRIP and value-based purchasing and contracting.

“S2AY has led an effective coalition of rural health networks in the state,” de Jung said. “It has also been a strong advocate ensuring that the perspectives of CBOs are included in population health work.”
