Sue Swift

Quality Improvement Advisor

Sue Swift serves as a quality improvement advisor on Common Ground Health’s practice transformation team. Her responsibilities include coaching, training and collaborating with primary care practices and other health care organizations. This collaboration aims to help primary care practices adopt a team-based approach to patient care centered on engaging patients as active, informed participants in their care.

Swift joined Common Ground Health in 2013, bringing a wealth of experience as a health care practice manager and as a project manager at Anthony L. Jordan Health Center and Highland Hospital.

As the OB/GYN/pediatric/special projects manager at Jordan Health Center, Swift managed program budgets, oversaw staff, authorized expenditures, hired staff and managed staff performance. She served in multiple roles at Highland Breast Imaging, Highland Hospital, including interim department manager, coordinator of patient and community relations, and outreach coordinator. She developed a keen understanding of the needs of underserved women and oversaw an outreach program to help them gain access to breast imaging services.

Swift earned a bachelor’s degree in communications from The College at Brockport, State University of New York. Her hobbies include bike riding, gardening and traveling with friends.
