Osterberg provides clinical guidance for care managers, specialized medical professionals located in primary care practices who assist patients with complex medical needs and chronic conditions. Osterberg helps care managers become trusted members of clinical practices, applying innovative interventions which improve outcomes for the most challenged patients. She also advises practices on building a team culture and focusing on the patient experience.

An advanced practice nurse, Osterberg brings more than 40 years of clinical and managerial experience to her position. She has served as a director of behavioral health for Clifton Springs Hospital and Clinic, a manager of mental health for Unity Health Systems and a program manager in long term care for Strong Memorial Hospital’s Department of Psychiatry and Behavioral Health. Early in her career, she practiced nursing in cardio and thoracic surgery at Strong and taught emergency nursing at Geisinger Medical Center.
