Quality Improvement Advisor

Darlene Colby is a quality improvement advisor on Common Ground Health’s practice transformation team. In this role, she coaches and supports physicians, nurses, and managers to improve patient care, efficiency and workplace satisfaction. Darlene joined Common Ground Health in 2014 as a clinical advisor for the organization’s Center for Medicare and Medicaid Innovation (CMMI) - Transforming Primary Care Grant program.

Colby utilizes her broad nursing experience and her background in business to meet the needs of the ever-changing medical landscape. As a nurse, she understands the clinical challenges facing medical practices, while her business background enables her to understand and support a practice’s processes and goals.

Colby was a nurse care manager for the groundbreaking Rochester Medical Home Initiative. She was responsible for introducing and championing patient-centered, team-based care for Lifetime Health Medical Group. She previously managed an operations center at Citibank.

Darlene Colby earned a master’s degree in business administration from the University of Rochester Simon School of Business and a bachelor’s degree with honors in nursing from the State University of New York College at Plattsburgh. A dedicated mother and wife, she enjoys traveling with her family, cooking, walking and biking.
