Data is at the heart of community health planning. And Common Ground Health holds the region’s most comprehensive collection of health data. It fuels our research. Reveals clear trends. And exposes realities about every segment of the population, including those most marginalized.

Convene the Right People

As the state-designated Population Health Improvement Program for our region, we gather diverse, relevant members from across sectors and from nine counties—Chemung, Livingston, Monroe, Ontario, Schuyler, Seneca, Steuben, Wayne and Yates. Common Ground Health works directly with key decision makers to devise solutions that they can put in place with their teams back at the office.

BUILD CONSENSUS

We help partners reach consensus on the right way forward, so everyone agrees to a plan of action—and can prevent duplication of services and coordinate their efforts for the best results.

COLLABORATE ON PROVEN STRATEGIES

We support our region’s effort to confront health challenges, from meeting the needs of our aging population to preventing lifestyle diseases like high blood pressure and obesity. Our collaborative approach to clinical innovation is recognized as a state and national model for improving patient care. 

LEVERAGE RESOURCES

Common Ground Health keeps partners in the loop about new government policies and seeks out shared funding opportunities. We connect and coordinate organizations around proposals. And we dedicate grant writing resources to increase our region’s chances of securing innovation awards.

TRACK PROGRESS

At Common Ground Health, we believe that the first step to solving a problem is to measure it. We work with organizations and county governments to develop health indicators, so that our region can monitor improvement over time and take action.
