By putting in place relatively simple new procedures, Tri-County Family Medicine Program, Inc., achieved a 5-percentage point decline in patients with uncontrolled diabetes.
This improvement is just one of many recent advances that earned the rural practice “exemplary” status from the New York State Department of Health—and an invitation to present at the National CMS Quality Conference in January 2019. Providers, patients and payers who attended the conference from around the country heard about clinical and performance milestones that have made the practice more effective, efficient, accessible and cost-effective.
Its goals are lofty, and its work is complex. Its timetable is urgent, but measurement of results will take years.
Despite these obstacles, the Rochester-Monroe Anti-Poverty Initiative is seeing early successes by harnessing the power of collaboration.
“One of the largely unheralded secrets of RMAPI’s success is in uniting groups working together to help them collaborate towards a shared aim,” said Wade Norwood, CEO of Common Ground Health. “Aligning the work of community partners to reduce poverty is long overdue, and it is something that RMAPI has helped bring about.”
